The early diagnosis of peripheral vascular injury is extremely important to avoid loss of limb and sometimes life. Dealays of six hours or more correlate with poor results. Peripheral vascular injuries may be diagnosed in either the primary survey or secondary survey of trauma initial asessment. Primary survey injuries are most often associated with massive hemorrhage and usually related to penetrating injuries. The remainder of vascular injuries shoud be identified in the secondary survey. This history is important to heighten the index of suspicion, but physical examination is the comerstone of identification of presence or arterial injuries.
