Frequently, when we are talking about epilepsy, we do a direct relationship with the gray matter; however, the role of subcortical areas is a very important component and sometimes the most important generator factor in some types of seizures, as occur in Myoclonus epilepticus or in absent seizures. Also, not only positive phenomena are involved in the semiology of the epilepsy, a lot times one seizure may start with a imperceptible decrease in the muscular tone or by a slight jerking followed for a tonic contraction of the four extremities (as occurred in Lennox Gastaut syndrome) and the physician might classify erroneously as a tonic seizure let without attention the reality: that the beginning of the seizure was atonic or myoclonic, and this differentiation is really important in the prognosis, talking about epilepsy surgery. Hereby, we are presenting the pathophysiology of partial and generalized seizures, as a tool for neurologist and epilepsy surgeons.
