Discussão dos impactos do nexo técnico epidemiológico previdenciário

Resumen

Background: In 2007 it was implemented by the Brazilian Social Security Institute (INSS) an epidemiologic tool to characterize work-related disability (NTEP). Its application has increased 152% the notification of work-related accidents and occupational diseases benefit. It’s necessary to study this new tool that directly impacts the relations present in the field of Occupational Health. Objectives: Discuss the application of the NTEP by the INSS and possible improvements in its application. Methods: Three hundred four medical expert reports were selected issued at a Social Security Agency in São Paulo city, between october/2008–march/2010, which resulted in work-related accidents or occupational diseases benefit by applying the NTEP. Correlated diagnosis established by medical expert and occupation described in the report, standardized by the ICD 10 and by the Brazilian Classification of Occupations (CBO), respectively. Results: About 186 causal relationships were established by NTEP. The most frequent diagnoses were musculoskeletal disorders and most frequent occupations were related with service sector (tertiary). Conclusions: The application of NTEP apparently caused changes in the epidemiology and increased diversification of work related diseases. Impacts were observed on Occupational Physicians and Social Security Physicians routines and to workers. It is recommended revision of the statistical basis of the technical nexus; INSS investment in training for its staff of Medical Experts; use the Brazilian Classification of Occupations as a variable in an expert evaluation; and systematic inspection of the workplace by the Social Security Medical Experts to evaluate the consistency causal etiology suggested by tool.
